At Help and Outreach Point of Entry (HOPE), we know that lasting change happens when organizations work together. Our mission is strengthened every day by the dedicated community partners who join us on-site to provide essential outreach, education, and support to those we serve.
From legal aid organizations to workforce programs, recovery services, and mental health organizations, these partners bring vital resources directly to our day shelter and outreach events — meeting people where they are and helping remove barriers to care.
Lower Shore Health Insurance Assistance Program (LSHIAP) provides free, confidential help to residents of Wicomico, Worcester, and Somerset counties in understanding and accessing health coverage. Certified navigators assist by reviewing eligibility, renewing coverage, and connecting individuals to additional community resources. LSHIAP’s goal is to remove barriers to care by making insurance easier and ensuring that individuals and families have the coverage they need to stay healthy.
Tidal Health’s Community outreach programs include those that provide health education and health screenings to our community. They also include the many activities in which TidalHealth is a partner in closing gaps in care and improving the overall health of our community, whether through improved outcomes or through prevention or disease management.
The American Job Center (AJC) Mobile Unit is a community outreach program utilizing the latest technology to deliver on-site employment search and career services. The AJC mobile unit is outfitted with a generator and seven computers with cellular Internet access making it a truly self-contained employment resource.
SWIFT (Salisbury/Wicomico Integrated Firstcare Team) is a mobile health program which plays a vital role in opioid overdose intervention and prevention, connecting individuals with follow-up care, primary care providers, and other needed health services.
Diakonia’s Rapid Response Team provides real-time, on-the-ground outreach to individuals experiencing homelessness across Wicomico, Worcester, and Somerset counties. Dispatched by law enforcement, hospitals, first responders, and community agencies, the team offers immediate support such as food, resource navigation, and same-day referrals to housing, rehabilitation, and mental health services. By building trust and reducing barriers — including for those experiencing “hidden homelessness” — the Rapid Response Team connects vulnerable individuals to critical resources and pathways toward stability.
Maryland Highway Safety Office – Community Engagement Program works to reduce traffic injuries and fatalities by partnering with local organizations to deliver education, outreach, and safety resources directly to the community. Through initiatives focused on pedestrian and cyclist safety, impaired and distracted driving prevention, and proper seat belt and child safety seat use, the program empowers residents to make safer choices on the road and builds stronger partnerships to improve transportation safety across Maryland.
