Projects for Assistance in Transition from Homelessness (PATH)

​​​PATH Program helps connect adults 18 and over who are experiencing homelessness, and who have been diagnosed with a serious and persistent mental health disorder, to link to benefits, entitlements, housing, long-term treatment and supportive services as needed.  Case management and peer support are offered through this program.
