Light Case Management Services
While we do not provide full case management, we are able to assist clients with a variety of supportive services, including:
Assistance with Applications: Help completing online and paper applications, or coordinating appointments with agency representatives as needed.
Client Advocacy: Calling customer or member service lines to assist with benefit questions, replacement cards, and other general inquiries.
Homeless Verification Letters: Providing documentation for unhoused individuals seeking services from organizations that require verification of housing status.
Supportive Counseling: Offering short-term, solution-focused support within the scope of our services.
Access to information
We offer brochures, flyers, and contact information for many local organizations to help connect you with additional resources. HOPE also hosts regular community outreach events β check our calendar for upcoming opportunities to meet and connect directly with these organizations.
Referrals
We are able to process referrals for the following:
Rapid Rehousing Program (RRH)
Provides rental assistance or helps clients to access rent subsidies. Rapid Re-Housing programs always provide short-term rental assistance and services, with services ending once rental assistance terminates. As with the Housing First approach, the goals are to help people obtain housing quickly, increase self-sufficiency, and remain housed. The Core Components of rapid re-housing (housing identification, rent and move-in assistance, and case management and services) operationalize Housing First principles.
We participate in the local continuum of care (COC) and the Homeless Management Information System (HMIS) to track client services, program outcomes, and tri-county data on homelessness.
